Transition

Transition or change can happen at lots of times in people's lives. When they leave school or college, a job or centre or move to a new home. Change and transition can be a stressful time and make mental ill health more likely. Planning for any changes can help reduce the problems.

One of the biggest changes in people's lives is the move from education into the adult world.

This is especially true for young people with learning disabilities and their families.

A whole range of services that have supported them since birth stop and new relationships have to be made.

This period of transition must be managed effectively to reduce the stress and emotional problems that can occur and ensure smooth and planned transition into new settings and services.

Other changes can have effects on the individual that may not be obvious. Changes to supporters, services, others using the service, the methods of getting to/from services. Check the website of another ARC project for more information

Moving On Up

The Moving On Up project offers an easy to access and fun to use resource for young people moving from children's to adult settings and services. There are sections on Health, Education, Employment Housing and Free Time.
